Incomplete place names
I'm researching in Solothurn canton Switzerland where there are parish registers and probate records online. I notice people add a source for a birth, death or marriage for a person. They fail to put the place in because familysearch does not provide it for them. Most of the parishes you actually have to look at the document and look for it. So now there is a growing number of people with sources, but they don't have an exact place name. The default place name that familysearch provides is 'Switzerland' That is what people are using. Your data quality score should take that into account

I forgot to add, These are German records, so people are adding names from the indexes without looking at the records. I don't think many of these people can read the basic parts of the documents.

This record group is Solothurn Canton as found on the familysearch wiki. There are links to the individual parishes that have been created. These parishes pages have direct links to the (images) records. Kriegstetten is perhaps the most active parish.
