Birth Record, Czudec Roman Catholic Parish, house no. 10 (could be #16), dated 6th May 1835, baptism 7th May 1835, Sophia (Zofia in Polish) Ruszala, legitimate daughter of Michael (Michał in Polish) Ruszala, farmer/horticulturalist, ? ? ? (I think this part is just talking about the midwife and the baptizing priest), and Marianna, daughter of Adalbertus (Wojciech in Polish) and Anna Gierlak, also a farmer Witnesses: Joannes (Jan in Polish) Zybura and Theresia (Teresa in Polish), wife of Joannes Rozborski.

If you have a link to the actual document you could try looking to see if the note starting with "Obstetrics" looks the same on the other entries. If it looks different, let me know and I can translate that section too.
