Translation needed for German birth record of Siegfried Cohn
On the birth record of my ancestor, Siegfried Cohn, G2QH-YSH, in Memel, Lithuania on 31 Oct. 1914, I can read most of the record. Is the father's name Selly or Sally? However, to the right side of the form is information I can't read and need translated. Could it be a marriage record? It's in German. ancestry.com/discoveryui-content/view/36129:61298?indiv+try&h&db

I read the father's name as Sally, not Selly.
The information on the right-hand side of the page says Siegfried was adopted by the merchant Ernst Hanff and his wife Magda Hanff nee Eisenstädt. According to the adoption agreement, Siegfried would no longer use the surname Cohn, but rather Hanff only.
