Could someone assist me translating the information about Josef Voráček's mother, Alzbeta (birth of Anna on 9 November)? I cannot read her surname, nor can I recognize whether her father is mentioned in this record.

The surname seems to be Witha. It is quite unusual, but still present in south Czechia https://www.kdejsme.cz/prijmeni/Vitha/hustota/
