translation request please

I can transcribe part of the first sentence-except for the first name for ---Czerny and can't interpret the second sentence at all.

Anno 1757 Die 17 January january sepultus est Panly??? Czerny ex pago kogakovitz anslion??? 62 annorum.  Non e proviso qvia?? repertis?? est in Silva Mortuary??? qui est arbore ???

In 1757, on January 17, Panly??? Czerny from the village of Kojakovitz anslion??? was buried at the age of 62.

Name also occurs in a seignorial register for 1759 Trebon estate, village of Kojakowitz list- might be Pavel

    Take a look at this Wikipedia article: https://en.wikipedia.org/wiki/Plural_form_of_words_ending_in_-us Some of the f looking ending on some words is a üs. For example after the vitz on the second line Rusticus= occupation farmer. The priest is not using the diacritic for u- ü,

    Anno 1757 Die 17 January sepultus=buried Paulus Czerny Rusticus= occupation farmer 62 annorum (of years) from the village Kojakovice. Non e proviso qvia repertis est in Silva Mortuary qui est arbore= Not from the provision that is found in the Forest Mortuary which is a tree (I think it means he was buried in the forest among the trees) (not positive)

    It does help, especially for the first name. I used google translate, too, but I couldn't transcribe the second sentence the result didn't make much sense. I did have four years of high Latin, but my main issue is reading the handwriting if it's something that doesn't follow a pattern to make a good transcription.
