City is not a City
At the time of the 1915 NYS Census, Mary Ellen Kelly Fritz was living in New York City. New York City, New York, United States is recognized as a standard placename in the FSFT, but her score summary indicates the residence conclusion is missing a City.
The same applies to her place of birth. Montreal, Quebec, Canada is a standard city placename, but the Summary shows it as missing a city.

Could it be that the City/etc. is requiring a four-part standard "city, county, state, country" and your examples only have three items listed?

But my examples don't require 4 parts.
In fact, if you use 4 parts on the city of Philadelphia, in the county of Philadelphia, in the state of Pennsylvania, United States, you'll get an entirely unwanted and incorrect display.
