Hi, I assume Sanit. Soldat probably stands for Sanitätssoldat, meaning he was assigned to a medical unit. I can't decipher what it says under Sonstige Lehranstalten but it probably has something to do with it.

That may read as 'Bezirks Commando Nummer 4' (District Command No. 4).
