It took place in the city of Mława on the fourth (sixteenth) of March 1899 at ten o'clock in the morning. Jan Kolakowski, thirty-six years old, and Leopold Kolakowski, fifty-five years old, both farmers from Mława, appeared and announced that Eufrozyna Blak, forty-two years old, had died in Mława yesterday at eight o'clock in the morning. She was a widow born in the village of Stryjewo Małe, Łysakowo parish, living in Mława, daughter of the late Alexander and Viktoria nee Piotrzkowska, husband and wife Kolakowski (Aleksandra i Wiktoryi z Piotrzkowskich małżonkow Kolakowskich), farmers. Upon certifying the death of Eufrozyna Blak in person, this act was read out to the illiterate present, I only signed it.
