Please help with details in baptismal certificate
I have a photo of this baptismal certificate, but I could not find the corresponding record in the archive church books. Can you please tell me what this two letters in line "Tomus" mean and what is the baptist's name (and other clues that could help if you have any ideas)? Also, is it possible that there is no such record at all (except for the case when the book was not preserved, of course) Thanks in advance!

Hello. The two letters look like T.D. to me, but I'm not sure what they mean.
Name of the priest is Jan Baptista Sladek
There is also a note on the bottom saying that Milada Soukupová left the roman catholic church. The whole sentence is: Milada Soukupová leaving the roman catholic church was reported.
Also she was born in 1918 but this certificate seems to be from 1931
