Translate my great grandfather's Italian birth record
I recently found my great grandfather's (Mariano Contessotto) birth record. If anyone can help me translate it I'd really appreciate it. I'm actually going to Italy this summer and will visit the town of Montebelluna where he was born. Any information I can glean from this document will be really helpful. Thanks!

Here's the genealogical information from the document (if you would like a more complete translation, let us in the community know! Someone else would be able to do it more justice than me):
Reported in the year 1904, on the 21st day of August, at 10:15am in Montebelluna by Santolini Giuditta, age 29, citizen of Montebelluna, daughter of Gaetano, who said that at 9pm on the 18th of the current month [August], a baby boy was born of Amalia Pivato, wife of Luigi Contessotto, age 35, son of Tiziano?, the baby being named Mariano.
The rest of the document just lists the witnesses from what I can tell.

Thank you so much! It looks like there is also an address listed. Can you tell what those few lines say? I'm actually going to Montebelluna this summer and would love to find this plot of land (even though I'm sure something else is built on it now).
