Newbie needs help with a translation, please

Hi
I'm new to Czech records. I'm looking for a Frank Krulish whose parents may be Frank & Marie (mentioned on a death record without the informant's name).
Can someone translate this record? Once I know how the record is written I can take off from there. It's how I learned to do German records.
