Need help translating czechoslovakian

Name and surname, his status, his job, father and mother, birth place, house number, region. Whether father of illegitimate child admits to it and wants to be „written in“, it is supposed to happen in person with two witnesses and he needs to sign it and so do the two witnesses:

First column lists his name, job (butcher), where he's from (Kladno) and parents: JOSEF ŠKARDA, blacksmith from a place I can't read properly and ANTONIA nee FIŠEROVÁ from Tuchoměřice.
Second one lists her name (ANNA PANENKOVÁ), where she's from (Kladno) and parents: ANTONÍN PANENKA (job I can't read) from Kladno and BARBORA ŠKIRITOVÁ/ŠPIRITOVÁ from Braškov. So no, Antontia Panenkova is not her mother, but it's Antonín Panenka, her father.
Not sure what the third one is supposed to be about but it lists a person's name and her husband and then another person on the bottom with his job and where he's from.
PS: the language is not czechoslovakian. There are two languages, Czech and Slovak, very similar but not the same. This one is Czech.
