Hi! Me again with one more translation request, please.
I have attached a death record for my ancestor, Ondrej Lieskovsky. It has taken me a while to make progress finding him because the information I have for him is spelled Andre Leskovski. He is record number 22 on this document, but I am not able to read the column headings to know what I am reading.
Would someone be so kind as to please translate the column headings for me in this record?
