Note: I am basing the year of birth 1875 on the Baltimore Health Department stamp in the upper right corner of this document.
Translation:
No. of child of mother: 9th child.
(1) Gender: female.
(2) Race or color: white.
(3) Date of birth: 17 August [1875].
(4) Place of birth: Belair Roenere [perhaps street names within Baltimore city?].
(5) Mother's full name: Katerina Helldörfer.
(6) Mother's maiden name: Katerina née Sauer.
(7) Mother's birthplace: from "Burgrub" in Bavaria [possibly Burggrub in Bavaria, of which there are five villages with this name].
(8) Father's full name: Johann Helldörfer.
(9) Father's occupation: cooper.
(10) Father's birthplace: from Reundorf? in Bavaria [there are two villages in Bavaria called Reundorf].
Name of medical attendant: Walbruga Müller [typically the first name is spelled "Walburga"].
Address [of medical attendant]: Sterling St., no. 165.

Thank you so much! I am indexing the birth certificates in Baltimore City and have about 6 more in German that I cannot decipher. Since I only need the parents names and date, I will make sured the entire translation is entered into FamilySearch tree and Ancestry. Here is another from the same day, same midwife:

Mother's maiden name: Kathrina Schüfer. [since the names are the same, this would indicate that the informant didn't know what the mother's maiden name was and simply repeated her married name].
Father's full name: Georg Schrüfer.
My comments: I am transcribing the names exactly as written in this record.
