First page indicated persons could not be merged because they had already been merged. When clicking on the surviving person, that was the deleted person.

Yes, this can happen when doing a series of merges and one loses track of what has already been done. It will only happen if merging by ID.

For example, say you want to merge AAAA-AAA, BBBB-BBB, and CCCC-CCC into a single profile.

If I first use Merge by ID to merge BBBB-BBB into AAAA-AAA, leaving AAAA-AAA as the surviving profile while I have BBBB-BBB in a separate tab, then go to that other tab and without refreshing the page try to merge CCCC-CCC into BBBB-BBB with BBBB-BBB as the survivor, I would get that error because BBBB-BBB had already been deleted in the first merge.
