It took place in the municipal office in the city of Szadek in Kalisz Voivoship in the county of Sieradz in the office of the municipal registrar on 10 February 1825.
Andrzej Powroz of the 4th regiment mounted riflemen, 1st squadron, a blacksmith by trade, a bachelor, 24 years old, personally presented himself, and in regards to his pending marriage to Miss Julianna Kapcinska who lives in the city of Szadek, requested that a Record of Acquaintanceship be drafted in lieu of his birth certificate.

In cases where an official birth record was not available for the bride or groom, an affidavit of identity called an “Akt znania” (literally, “Act of Knowing”) was provided. In an Akt znania, testimony regarding age, place of residence and marital status was provided by witnesses who were well acquainted with the individual.
