I'm interested in the last record on the page for Rosalie Zacek. I know her parents are Josef Zacek and her mother's name is Anna. The note under her name is probably about her immigration to America in 1852, but not sure. I can tell her house number, but I'm unclear what the name of the city is. Any help would be appreciated.

Re: City: It looks to me that the place's name is 'Predmestj'. According to my dictionary, 'mesto' means city/town and that's a location in quite a few of those entries. The word for suburb is 'předměstí'. I think that's close enough, so the answer is probably that the family resided in Březnice or Preßnitz (which is also the place the register concerns according to its description.)
The mother is Anna, daughter of (Matej/Mathias) Bradac, ?(probably his occupation), of Kletic(?) and Anna Kaubek(?) of Wostrowa.
The note concerns 27 March 1852 (No 114); I'd have assume 'Bap' to mean Baptism. It may be worth checking out to be sure.

@StH31 Thank you for your assistance. I was able to find Rosalie's parents marriage record in Kletic. I believe Wostrowa is right as well, now I just need to see if I can track down Rosalie's parents birth records.
I also had thought Bap could mean Baptism, but it doesn't make sense why she and her brother (b. 1839) would have been baptized in 1852. He has the same note under his birth record. I believe they both immigrated in 1852, so maybe the priest was documenting that he sent a letter that they were baptized Catholic? I have no idea - grasping at straws here. Their parents (Josef Zacek & Anna Bradac) marriage record lists they were both Catholic and both Rosalie and her brother practiced the Catholic faith after they immigrated to the U.S. So I can't imagine why they'd delay a baptism.
There is a family story that Anna Bradac may have been a gypsy. It could make sense if she had been baptized at a later date, but not her children as she was already a documented Catholic.
