The picture is too blurry to read. I looked up the film # and image #. A readable image can be found here:

The page is birth/baptism records but sometimes deaths are also recorded on baptism records. I didn't notice any on this page but might have missed it. Which record were you interested in?

Greg is correct. The sixth column is the birth date. The 9th column is the baptismal date. I don't see any death dates. In some cases the pastor went back to the birth record after a person passed on and added the death date but this record doesn't appear to be one of those.
It is helpful to download the document, crop it and mark the part you want translated. Keep the file large and show the whole page so the translator can compare the letters in your entry to others on the page. I put it into photoshop and enhance the image so it is a clear and crisp image. All the best.
