Question about name of mother in birth records - why two different names?
When I found two birth records for Edmondo Vittorio Alberto Mauro (one from State of Illinois and the other from City of Chicago 2 pages entry 18025), I was surprised that his mother had 2 different names i.e. Maria Valvo and Annantonia Breglia in the State of Illinois record and only Annantonia Breglia in the City of Chicago record. Edmondo's baptism record (second entry on right) shows his mother was Maria Valvo and his death record shows Maria Galva which I think was misspelled with a G. I can't find his marriage record. However I think he was in the 1940 census living in Oak Park, Cook, Illinois and married to Belle?
I included his World War 1 registration card because it shows the birth date as 23 Sept. 1885 which is different from 22 Oct. 1885 in the birth record, 24 Sept. 1885 in the baptism and 22 Sept. 1885 in the death record. I'm okay that his birth date is off a bit because of the time period being the 1880's and I expect it to vary a little.
FYI: for some of the records I included the entries and the entire page for reference.
Question: In the State of Illinois birth record, what do you think is the meaning of line 6 "Full name of mother and maiden name" Maria Valvo = maiden name Annantonia Breglia? And that the City of Chicago birth record shows the mother's name was Annantonia Breglia? The other records show his mother was Maria Valvo.
I posted two other questions: Maria Valvo's birth record and the marriage record for Edmond's parents (Andrea Mauro and Maria Valvo). Please see those posts because I want to verify the name of Edmond's mother in those primary sources for her name and request the translations.

I will translate the 1853 birth and 1869 marriages you provided on those posts.
I have no idea why the Chicago birth record would give 2 different names for the mother. What's interesting is that it seems Andrea was the attending physician for his own son's birth, so it seems he filled this out himself. Perhaps he did not know what maiden name meant as there is no such concept in Italy, and so he figured it meant the midwife's name.
Nonetheless, I am fully certain the mother is Maria Valvo from Potenza province, Italy, and the father Andrea Mauro from Salerno province, Italy because church records are very reliable for this sort of information. As for the diverging birthdates, obviously there was some confusion as to the true date of birth, but this is totally normal for the time. I would think the real date is the 23nd of September 1885 since he was baptized the 24th of September and that Andrea made a mistake filling out the birth declaration, got confused as to which month it was.

I think that your suggestion that the father filled out the record and added Annantonia Breglia who could have been the name of the midwife, is a possible explanation. And I think a reason the wrong name is shown on the other birth record is because a clerk could have transferred the name from one birth record to the other birth record not knowing which one was the correct name of the mother.
I agree that Maria Valvo from Potenza province is the correct mother. Thank you for confirming that! And it's helpful to know that church records are very reliable for information about the parents.
Question: I think the baptism date for Edmondo could be the 15th of November and the birth date was the 24th of September. Is that how you read the dates?
