Can someone please help me with the first two lines (above the date)? The top lines are one clergy, and the bottom lines are the other clergy.
Also have a few holes that need filling. Feel free to correct any mistakes!

I may not be able to get all the translation of the lines above and below the entry, but here is the gist. The child was actually baptized in the Heiliggeistkirche in Heidelberg. This entry is an extract from the original book.
The first lines are:
Abschrift: Auszug aus dem Geburts und Taufbuch des evang[lisch] ??? Gemeinde zum H[eilig] Geist in Heidelberg,Jahrgang 1858 No. 47
Copy: Extract from the birth and baptism book of the evangelical [Protestant] parish of the Holy Ghost in Heidelberg, year 1858 number 47.

The first blank in your translation should read "durch Stadtpfarrer Theodor Plith". The name can be seen much better in the Heidelberg church record. The status for Karl Kübler is Kirchendiener or church sexton.
The copy also includes the name of the pastor of Heiliggeist, J. Holtzmann.
I believe that the final lines are just an attestation of the entry by the Pfarrer Burck? of Neuenheim.
Note that the church books on FamilySearch are duplicates that were copied from the original records in the church. Copies of the original books are available on Archion, if you have access to that. You may notice some differences. For example, the original Neuenheim entry has a note that Ludwig Michael died 19 August 1859.

When I first saw the top line, the only word I could make out was Gemeinde. I knew that the word before it was evanglisch.
Crazy thing is I have the original, a copy with the comments on bottom & this one. This is the most legible with comments.
Heiliggeistkirche in Heidelberg is a step up from the local parish church. Of course, 70% of my folks were baptized at home. Lots of my people are in a marriage where one spouse is Catholic & the other is Protestant.
Not ignoring the comments has added to my German vocabulary. And helped me know what I can expect next time I encounter them.
