If you are referring to a living person for whom you're looking for a priesthood ordination, and you say it was "never digitized," does that mean that the person's formal membership record (to which the ward clerk or bishopric have full access) also does not show it? And has anyone (in leadership) talked with Membership Records to see if they can determine anything?
Also, if the ordination is going to take place outside of the home unit of the individual performing it, that person's bishop is supposed to authorize the person to perform an ordinance outside that unit, and the presiding leader where it is to take place should verify that. (Note: I have not researched this in the brand new handbook, just in case there has been some brand new change in this requirement.)
Beyond that, I'm not understanding where the microfiche comes in. Sorry.

I too am a little confused. I have never heard of ordinations being filmed. Do you have a film or fiche number we can check on?

If you go to the Church History Library, not the FHL, you can request a copy of priesthood ordination and line of authority records, but you have to prove membership, relationship, etc. Don't know if you can do it over the phone or via email though.

Thank you all for taking the time to help me out. Here is the official answer I received from the church historical department in case this comes up in the future:
Go to www.lineofauthority@churchofjesuschrist.org.
*Put PLA in the subject line. This stands for Priesthood Line of Authority.
*Request the form for a replacement and provide details of where you want it sent.
They will contact you via email. There may be additional questions they need answers to before providing the document. There respond pretty quickly I'm told
