Would someone have time to help me understand the information in the margin of this birth/baptism record? Perhaps it is an address of the family?

The first note has something to do with her name and hopefully someone else can give you a better answer than mine. [?] name of Silvana de Medeiros Carneiro at her [inscription?] in the casa do Povoa on 14 May 1980.
The second note is about her death on 30 October 1989 in the parish of Lomba da Maia in Ribeira Grande.

On the margin of the first image it says she goes by the name "Maria de Medeiros Carneiro" as known by her application in Casa do Povo, on May 14 1980.
Then it says a new Cedula Pessoal was given to her, number 842919 serie C, on the 14th of May 1980. This Cedula was a portuguese document that served as an ID. On the margin of the second image it indicates her death as on 30th October 1989 in the parish of Lomba da Maia, Ribeira Grande. Record number seems to be either 135 or 136.

Thank you very much. This information will be helpful. However, the records I know of stop at 1910. Where do I find death records for later years, such as 1989 for this specific record?
