    All this is official according to instruction of Radziechów, County Office, dated 11/12 331 L. 5233

    3. Circumcision on: 23rd April 1878, Radziechów, in Synagogue.
    4. The Infant: Kohos Hersch of 2 names, a male infant
    5. Infant Illegitimate (crossed out?)
    6. Name of Father, his surname, occupation, place of residence: Majer Wolf Glaicher, a farmer, owner of a house in Radziechów.
    7. Name of Mother, her surname, occupation, her residence, and residence of her parents: Hana Kamelmacher, farmer’s daughter of Mozes and Keila Kamelmacher from Radziechów.
    8. The signatures, occupation and place of residence of the godparents or witnesses: Gerszen Gleichler, and Schulem Laurin.
    9. Circumcision performed by Pater Golden.
    10. Nurse: F. Balah.
    11. If stillborn infant: not applicable
    12. Comment: I know in person Meier Wolf Gleicher, who states that he is the father of Meier Wolf Gleicher, Schulem Laurin.
