Jana Chrzciciela, of Bronislaw Suligowski, who was born in Radom on the 5th April 1877, christened on the 14th August 1877, son of Felicjan Suligowski, a doctor, resident of Radom, 30 years old, and of his wife, Bronislawa, born Cywinska, 25 years old. Witnesses: Leon Cywinski, a pensioner, 59 years old, and Wladyslaw Siemnicki (?), a church servant, 30 years old. Godparents: Leon Cywinski and Karolina Sierszowska (?).
Note on margin, re. Birth Record No. 390, Bronislaw Suligowski died on the 18th April 1950, Civil Records Office of Radom. Death Record No. 511/50 year.
