If you get a smartphone app called Google Lens, you can hold it up to the document and it will visually translate what it can. Although it is not doing so well right now on your document, it's better than nothing, and it's instant.

I can read a little Slovak because I studied Russian. I have been doing my own Slovak genealogy research. Just wanted to tell you that I am not the best person to be reading this for you, but I can help. I used Google Lens and Google Translate on my desktop. Both Slovak and Hungarian.

This document is from 1903 and is in both Slovak (top line of each section) and in Hungarian.

This document says Birth Certificate Extract. It isn't a church record. It is a civil record. After 1895, the vital records in Slovakia were ordered to be kept by civil authorities (town hall, etc.).

Here is my translation, with just the Slovak words. (My keyboard doesn't have accents set up.)
