    Translated from Polish:

    Death Record No. 4, Czudec, of Wojciech Gierlak, who died in Zaborów on the 26th March 1856, he lived 28 years, he was son of Józef Gierlak, a farmer, and of his wife, Apolonia Gierlak, born Rykała.
