I know that the FS website has been having some glitches the last couple of days, so perhaps this problem is an artifact of those issues. Vital Audon M15M-XHD has 4 sources attached; 2 are indexed, while 2 are not.

The PQS calls out a conflict "This person has no indexed sources."
