Instructions say to add an entry for spouse when included on the same line, i notice that on the sheet i am doing the spouse has already been included on a separate line. As the lines are alphabetical by given name I didn't realize that at first. That also clears up the fact that the spouses surname is not assumed.. On the example Andrew has wife Philemene. Scroll down the page to the P's to find her and Andrew. All info is same as info in example line.

Do i continue as instructed.??

    While it has the same information it is not a duplicate. Go ahead and index both lines.

    I need further clarification on this, please. When the entry reads O'Neil, Thomas, wife of Eleanor, do I need to do an entry for Thomas O'Neil and then an entry for Eleanor O'Neil. I assume I do. Is this correct?

    You are correct. However Eleanor's surname would be Blank (ctrl b) as we are not to assume a last name is the same.
