US, Wisconsin—Birth Records, 1907–1911 [Part A] [M398-GWJ], Multiple Birth Help?
Hello! On image 2, he is a twin right? It says he's the first born, but there are 2 children living, and triplet and other are crossed out, but not twin. Also, if he is a twin, do I have to make another entry, or is that only if it gives the name of the twin?

Looks like it is a twin, and this child is the first born. Project instructions regarding twin
- Index information about twins or other multiple births as separate entries, even if they were recorded on a single line or in a single document. In the case if they were recorded. In this case I believe there is no record of the second child. No need to create separate entres.

Can you share the batch information? It is hard to know without actually seeing what you are seeing. If it is a twin and info for both are on the same image, then you do need to create another entry. If it only gives information (name, birth time) for one, then you would not create another entry.
