Translation of a line in a household survey.

Can anyone translate for me all of the markings in line 6 (Anders Olsson) on both pages of this household survey?

    The column showing profession says he is an Apotekdreng. Apotek is apothecary, showing he is an apothery worker. Column 3 and 4 shows his birth date; 27 April 1890. Column 6 shows he has been vaccinated against smallpox, column 11 shows a gk - rates his christian knowledge - probably stands for good (god kunnskap). Column 12 shows he has taken communion. Column 15 is about his obligatory military service. frik is probably short for frikjent, which means acquitted. Sorry, I am not familiar with the numbering system shown for that service.
