Why is my father's ordinances not available?

My father's name was:

Norman Dodelin McKee

1 November 1918 – 6 December 2002

ID # KWCR-HC9

My father was excommunicated and later received permission from the first presidency to be baptized. I performed the baptism and confirmation. I do not remember the date over 20 years ago that this occurred. His bishop at the time was Gary Huber of the Garland 1st Ward, Garland, Utah Stake. Shortly after baptism he was moved to an assisted living center called "My Home" in Tremonton, Utah. His baptism and confirmation does not show in family tree. I do not have a physical copy of the baptism/confirmation certificate. It occurred about 1999. I would like to make sure my father's records are up to date and complete any needed ordinance work for him to progress.
