How can a patron reserve ordinances for his friend Nicolas Alonso Victoria (LT7C-GTJ)

Patron is not related, but he has permission from the spouse of Nicolas. He lived with their family for several years. Written permission can be obtained, but what format is acceptable? Patron is Jesus Alvarez. I am a consultant helping him today. Thanks, Boyd Christiansen

    You will notice in this article the exact wording that should be used in the written permission:

    I, (name of person giving permission), give (your name) permission to do temple work in a Church of Jesus Christ of Latter-day Saints temple, for (name of deceased), who is my (relationship to person giving permission).  (Signature of person giving permission)
