I am requesting permission granted to perform the proxy baptism, confirmation, initiatory, endowment and sealing for July 20, 2021 at the Raleigh North Carolina Temple. Thank you.

    Please use the above information to contact the Raleigh Temple for the ordinances you wish to perform. You must log in to the temple to request completing ordinances.

    If you need instructions on how reserve ordinances, they are below.

    If you want to reserve temple ordinances for your ancestor, use Family Tree. If you do not have an ancestor ready for ordinances, use Ordinances Ready.

    Reserved ordinances appear on your temple reservation list, where you can print or share the ordinances. If you cannot complete the ordinances within a few weeks, you can share them with the temple or others.

    Note: Temples can resume printing family name cards during their phase 4 reopening. If your temple is at phase 3 or earlier, please print cards before you come to the temple. After you complete an ordinance, the temple records this information in FamilySearch. Temples do not return name cards to patrons.

    Reserve ordinances through Family Tree

    Steps (website)

    1. In Family Tree on the FamilySearch.org website, display the relative whose ordinances you want to request. 
      • If you are on the person page, click Ordinances, and then click Request.
      • If you are on the landscape, portrait, or descendancy ​view, click a green temple icon.
    2. Click to uncheck any family members for whom you do not want to reserve ordinances. (You must request all of the ordinances.)
    3. Click Request.
    4. Carefully read the Church Policy.
    5. If you agree, click Accept.

    Reserve ordinances through Ordinances Ready

    Steps (website)

    1. Click the Temple tab that you see on most FamilySearch pages.
    2. Click Ordinances Ready.
    3. Tap beside the ordinance you want to perform.
    4. Click Take to the temple.
    5. Follow the prompts on screen to print the name card. Or write down the Family Ordinance Request number and take it with you to the temple. A temple worker can use the number to print the name card for you.
