Incorrect ordinances

Roy K. Lemley ID # KWJJ-72B (Changed his name to Paul LeRoy Winthrop)

His baptism and confirmation dates on Family Search show it was completed on 13 August 1933. This information is incorrect. He lived until the 6 March 1939 and was never baptized while he was living. The date of 13 August 1933 is significant for the family because three of the children of his family were baptized on this date. One of these children, Paul LeRoy Winthrop, Jr., was named after his father.

After the death of Roy K. Lemley in 1939, his family had his baptism and confirmation done in the Mesa Arizona temple on 12 April 1941 and 07 May 1941 respectively.

My question is, how do I get this correction made on Roy K. Lemley's ordinance records?
