German Translation Needed

I need the following Birth Record translated for Helena Noe, born 19 May 1874, Balsbach, Baden, Germany.  I think it is No.4 on the attached sheet. Her parents were Mathias Noe and Margareta Blatz. There are almost 2,000 group members, many that have lots of experience researching their families, speak and read German, and are willing to help.
