Need advice for this indexing record

I'm indexing Chicago Birth records and I came to this one which shows corrections for the document before (I just indexed). What do I do with this page/entry? Do I just go back to the other document and make the changes or link the two documents together?

Please help and advise me.

    I noticed in the Project Instructions (last icon at the top of your screen), the third example is an affidavit of birth. At the bottom on the right, it says, Note that this record should be indexed by itself. I would index it as is. It also says, Note that the incorrect information is not indexed. So it will be almost identical as the previous image. It will be good for the researcher to be able to see the original and the correction. There is probably some story behind the change.
