Registrar date for amended record

I am indexing an amended record. When it asks for the date that it was received by the registrar do I use the original date (June 4, 1909) or the amended date (August 10, 1960).

    @stephaniehardin1 you would use the date of June 30, 1909 as that is the date it was filed. You can see this at the bottom of the document. Please also see the first example in the Project Instructions which would explain this very well. We need to be careful not to confuse the date of birth with the file date.     I'm pretty sure you use the amended one and then check the amended box.
