OTHER Category in Web Indexing Reports

Has anyone had a problem where a name shows up under OTHER in the Web indexing report: The person listed under OTHER did not request to join the stake's indexing group nor did the stake receive a request from that person to join.
Years ago we had one person who appeared on various wards and stake reports in Hawaii but he lived in Utah. Same scenario. It took a while for FamilySearch to sort it out, but his numbers could not be removed. So that ward under which he was listed met its annual indexing goal very early.
Now it's happening again and FS has not responded to my query. So I'm reaching out to the Community.
