What constitutes a charge-out or amended document?

I am indexing US Birth Records, and I have come across records that have red ink changes to the information. The information varies from file numbers, registrar information, birth dates, and in this case the child's name. Do these count as amended or charge-out documents or should I just mark them like crossed-out or corrected information?

    I'm no expert. Having said that, I would definitely say this an amended record, so "Y" would be entered in the Ammended record space. I would index the child's first name as "Stefan or Andrew Lee" and his last name as "Homan or Holman".

    Please correct me if I am wrong all you indexing experts out there.

    I agree with Solson on how to index the name. In the bottom lefthand corner of the document, you can read about the legal name change made in 1943. So yes, it should have a "Y" to indicate this record was amended.

    Sorry for this delayed addition to my response. This is from the field instructions:

    A document is considered amended if any field was crossed out, if information was replaced, or if the word "Amended" was written somewhere on the document. I take this literally. Don't mark it amended if the actual form was crossed out but if the words that fill in the blanks were changed. I personally have never seen a "charge-out".

    Is the handwritten number in the top right corner considered a field? There is no place to index this number. Thanks

    The instructions don't distinguish a difference based on whether or not it is to be indexed, so i mark it amended regardless of the field. Perhaps others do not.
