Cannot "Replace" a parent

The PID was copied from the tree and pasted into the replacement request - and I also tried just typing the PID in with the same result . . . I also tried searching for the replacement with a compete copy of the name and birth details which failed to find a matching record.
Any explanation?

Two steps instead of one . . . but the "replacement" still has a fault.0 -

The child is added to both parents separately but the parents do not get added together in a relationship. I'm interested to see what the employees say about this.0
