Very hard to be sure due to the poor quality of the image, but my best guess is that says quarantasei, so 46 years old. Could it be quarantatre? Sure, but I think I see a dot over an i. Sad. I also believe the death record says Giovanna Clemente died in her parents' house after one hour of life.

I think it is 43 (quarantatre). Unfortunately there's no corresponding birth record to compare to.

Definitely a matter of interpretation. Since every instance of a lower case 't' in the document is crossed, and no such crossing is there, and since what would be the 's' is very similar to how the 's' in casa is formed, added to a dotted i, I am leaning towards quarantasei.
Of course, Diane, your next best approach is to find marriage records for Vito. Ages on marriage records are a bit more reliable than on death records.EDIT NOTE:
Page 3 of the marriage document for the Marriage of Giovanni Leone and Santa (Incerta, Bove) Capuccio/Capaccio (that you asked about in another thread) gives us the date of death for Vito Clemente, 2-Jul-1859. On this death record, his age is listed as 48. While often unrelaible, it does add credence to the age on Giovanna's death record being 46. My best guess at Vito's parents' names are Gaspare and Vita Marciglia.
