How to find the actual record of a book and page number of a Index?

This is a snip from an death index. On the far right you see 23, 30 which stands for "Book 23 Page 30". How to I find book 23 and page 30 in ancestry or familysearch? The link to where this line is online is below.

    Hello @EarlLaughter,

    This index was made by the Macon County (North Carolina) Register of Deeds, who presumably holds Book 23 as identified in this index.

    I checked both FamilySearch and Ancestry and it appears the records for the Macon County (North Carolina) Register of Deeds are not available online.

    I would suggest you contact the Macon County (North Carolina) Register of Deeds directly and asked them if they hold volume 23 and if so how you could purchase a copy of the death certificate on page 30 for Thomas O. Keith.
