A reason is not always entered. If the contributor entered a reason, it will show in the Reason box, just below the item.

For example,

I don't always enter a reason if I feel the source is self-explanatory.

You can also see the Reason(s) in the Change Log.

In the upper left hand corner of each section, be sure to have "Detail View" toggled on.

I too won't enter a reason if self-explanatory - most often because it is explained in the attached source(s).
