US, Arizona—Birth Certificates, 1880–1940: Combine Supplemental Birth Report? Surname spelling dif.

Wanted to get some thoughts on this one. In this batch, there is a birth certificate and a supplemental report of birth. I'm confused as to whether they refer to the same person, and need to determine this because project instructions seem to suggest they are indexed differently depending.

Some details:

-The name of the mothers match

-The birth date matches

-The father's and child's surnames are spelled slightly differently (Nelson vs Nelssen)

-The child's first name is different (John W vs Wallace C)

-Records are adjacent

Project instructions say "If a birth certificate and an adjacent image are for the same child, index all the information, including corrected information, in the entry for the birth certificate."

Three questions:

  1. Do I treat these two as the same person and index information on both records in the birth certificate?
  2. If so, do I include both given names separated by an "Or"?
  3. If I should only include one name, should I use the name from the birth certificate or the name from the supplementary report on birth?

    With a supplement and a birth certificate, you will combine all information from both images, separated by Or with 2 or more names and index them on Image 2 with Image 1 being marked NNED.
