US, Ohio—Naturalization Records, 1848–1966 [Part B] [MQGV-D5S]
Image 1 (and 2) have a COA. Can I use the the "Issue NO." in the bottom left hand corner for the record #? Also, what date should I use? This form is different than the sample given.

The instructions for Ohio are a bit different than those of other Nat projects:
If an image contains multiple documents on a single image and all of the documents relate to the same person, combine the information from all of the documents on the image into the entries you index in the data entry area, using the most complete version of the information.
Image 1 will have the first name as Peter Mathew or Pierre and the surname of Barbeck or Barbek with the record number and date coming from the COA; 4/2/1928. You will follow this instruction with Images 5 and 12. Image 13 is a duplicate of Image 12.
If you have a stand-alone COA that is like the one in Image 1, you will use the Issue No. as the Record Number and 4/2/1928 as the record date.0
