When indexing directories, if there is someone with a spouse's name in parantheses, do we index the person and their spouse as two entries? Or just do the one of the husband, with his wife's name listed under spouse?

The spouse is indexed in the spouse's name fields. We don't create a separate record for them. Their names will be listed in the Family Search index as individuals when published.

What we do not do is index the name of a deceased spouse. Under Project Instructions/What To Remember About This Project:

If a spouse is listed as deceased by the words "wid" or "widow of," do not index the spouse.
