Request for change of name

When there is a request for a change of name on a Petition for Naturalization, do I create a second entry for the person using that name as well, or only create one entry using the name given at birth?

    You do not create a second entry. You will index both names, separating the names by the word or.

    Ex:

    Given Name: John Or Jon

    Surname: Smith Or Smyth

    This instruction is found in the Field Help (purple question mark) in both the Given and Surname fields.
