Texas Death Certificates Indexing

I need help with a couple of this. I have been noticing that while I am in the process of indexing these records a few issues keep reoccurring. I have noticed in older death certificates that are written in, there is no record number to be able to look up funeral homes and individual records. I have also noticed that even some of these that do have record numbers, they either don't find a result or are under a different name. (Question: What can be done). Also, I have seen a few records that has two listed funeral directors. I have also noticed that some records do not have a funeral home record, but has a funeral home director listed.
