It would really be appreciated if anyone can tell me how this can happen?
The second one is mine done in June 2018.
Can this be reported to the technical teams by us at all? And how?
This is the absolute first time that I have come across this situation, and cannot believe that this is the only duplication of Unique ID numbers?
Kind regards,
Memory Smith

I'm not LDS, but those look like two views of exactly the same information to me.
Perhaps you'd get more knowledgable commentary if you posted your query in the Temple category.

I've looked at both images. One is the FamilySearch website and the other is your printed temple card and they show the exact same information. All ordinances are reserved Louise Smith.
Please share more details as to what you are seeing that is in conflict.

Okay, I looked at it more closely. Please go to the PID in FamilySearch and look at the Change Log. It looks like you and two other users have made changes to this person's record. Name changes and date and place changes. You will want to check through all the changes and sources that have been added since you printed the card to make sure what should be the correct information. Then update the FamilySearch website to be correct. If needed you may need to reprint the temple ordinance card.
