Permission to do ordinances

I would like help about requesting ordinances for a friend. I have received a signed permission from his closet relative.

I have completed all of those steps. My friend's name is Roger Bryan Cauthen. His Family Tree ID is GV3L-P6J. The signed permission is from his sister - his only living relative (besides a brother). What else do I need to do to proceed?

Would it be helpful to know that this is someone the missionaries were teaching and who was prepared to be baptized but died unexpectedly before his baptism date?
